Are you concerned about the wrinkles under your eyes making you look older? Many of our San Francisco patients come to us worried that these wrinkles are causing them to appear aged, but even young adults can have wrinkles, especially when they smile. In most cases, the real culprit of an aged appearance is the dark circles and bags under the eyes that result from volume loss. The good news is that fillers are an excellent solution for reducing dark circles and under eye bags by restoring volume and providing structure. Additionally, under eye filler treatments may also improve wrinkles as a secondary benefit.
While under eye wrinkles themselves are challenging to address directly, they typically are NOT the main cause of an aged appearance but rather just a small contributing factor. At Mabrie Facial Institute, we find that, in most cases, it’s dark circles, sunkenness, and bags under the eyes that are causing the aged appearance. Addressing those signs with under eye filler will support the skin beneath the wrinkles and give a brighter, fresher appearance. It may also improve the appearance of the wrinkles in some instances.
If your wrinkles form at the outer corners of your eyes when you smile or squint, we smooth these with BOTOX for crow’s feet. This can be done on its own, as part of our Upper Face BOTOX package, or in addition to under eye filler.

The result is hollowing of the tear trough area and sometimes bags—pockets of displaced fat. These contour irregularities and the shadows they create are usually the main reasons for an aged under eye appearance, not the fine lines and wrinkles.
With age, the skin undergoes a decrease in the production of structural proteins, including collagen and elastin. This causes skin to become thinner and less elastic, contributing to crepey skin texture. This is especially true of the extremely thin skin under the eyes.

Our exceptionally trained Injectors carefully introduce small, safe quantities of filler for results that never look artificial or overdone. At MFI, we use a blunt-tipped microcannula, which minimizes bruising and lowers the risk of complications. We place the microcannula just under the skin and use a fanning technique for precise filler application. We almost always stage treatment across 2 appointments.
At MFI, under eye filler requires a minimum of two appointments. This staged approach allows us to first establish structure and support and then make refinements for the best results. We start conservatively, then build and layer until we get it just right.

For the best filler experience, follow these tips in advance of your appointment:
Immediately after filler under your eyes, you may experience swelling, bruising, tenderness, and redness. These are normal side effects and are most apparent 24 to 48 hours after treatment. They can last several weeks but typically subside within 2 to 5 days. Most patients return to their routine activities the same day.

Since the skin under the eyes is very thin with minimal fat beneath it, the best filler for the under eye or tear trough area is a soft, moldable, hyaluronic acid filler. We use Restylane-L® to add volume under the eyes.
BOTOX is the go-to treatment for crow’s feet—the branch-like wrinkles at the outer corners of the eyes. That’s because they are dynamic wrinkles, meaning they are caused by repeated facial expressions. For finely wrinkled skin directly beneath the eyes, we find that filler, especially to smooth underlying hollows or bags, is the best solution. Once the volume loss is corrected, often the appearance of the wrinkles is improved.
Our patients usually enjoy results in this area that last a year or more. We recommend maintenance treatments approximately every 9 to 12 months so that you can remain looking your best indefinitely. The amount of filler required at these appointments is usually significantly smaller than during the initial stages of treatment.
