Rethinking the ‘Best’ Filler
Every day, patients ask us some version of the same question: “What’s the best filler for my lips? My cheeks? Under my eyes?”
It’s a great question, but it’s the wrong question. The truth is, there’s no single “best” filler. There’s only the best filler for each specific area of your face. Knowing the difference is one of the most important and least talked-about parts of getting filler results that look natural.
The Real Choice Isn’t Which Filler To Choose—It’s Which Kind
Dermal fillers aren’t one-size-fits-all. They vary enormously in thickness, firmness, and how they behave once they’re under your skin. Some are dense and structural, built to support heavier tissue. Others are soft and pliable, made to move naturally with thin, delicate skin.
Choosing the right filler is just as much a part of your result as your injector’s technique. Here’s how we approach this decision:
- Soft, pliable fillers go where the skin is thin and moves a lot. The lips are the clearest example: A filler that’s too firm here can feel unnatural or look lumpy when you talk, smile, or purse your lips.
- Firm, supportive fillers go where the skin is thicker and needs real structural support, such as the cheekbones and midface. These fillers are best when you’re restoring lift and volume rather than softening a line.
- Medium-thickness fillers tend to work well in transitional zones with little tissue coverage, like the nose or under the eyes. We use these fillers when we want to add some structure but also need the result to stay soft and undetectable.
Mismatched, Not Overfilled: Why Some Results Look Puffy
Patients sometimes come to us after a treatment elsewhere, describing a result that looks swollen, waterlogged, or puffy—even months after their injection. Often, this isn’t a matter of too much filler. It’s the wrong filler for that anatomic area.
Certain fillers are more hydrophilic, meaning they attract and hold onto water. In the right spot, that property can be an asset. In the wrong spot, particularly in thin-skinned areas like under the eyes, that same property can cause filler to swell, creating the puffy, tired look that patients are trying to avoid in the first place.
This is one of the most common corrections we perform, and it’s almost always preventable with the right product selection up front.
Why We Often Combine Fillers Within a Single Treatment
Because every face is different, it’s common in our practice to use more than one type of filler—sometimes several—in a single treatment plan. We may pair a hyaluronic acid filler with a filler like RADIESSE®, a calcium hydroxylapatite filler, depending on what a specific area needs.

Temples
Hollowing in the temples is a natural but often noticeable change that comes with aging. Restoring the natural contour between the brow and hairline requires a firm, structural filler, as well as a provider comfortable working in an area with limited tissue to guide placement.
Under Eyes
The skin under the eyes is among the thinnest on the face, leaving little room to hide heavy filler or poor technique. Placed correctly, a soft but supportive filler can soften shadows and smooth hollowing to brighten and refresh the eye area.
Nose
The nose has very little soft tissue to work with, and skin thickness varies significantly from the bridge to the tip. A filler here needs to hold its shape precisely, as even a small amount of spreading or shifting can throw off the whole profile.

Midface
As the midface loses volume, the face can take on a flattened or drawn appearance overall—not just localized hollowing, but a general loss of the fullness that keeps the face looking youthful. Nasolabial Folds
The crease that runs from the nose to the corners of the mouth moves constantly with expression, and it’s often as much a structural fold as it is a result of changes in facial volume and support. Lips
